ABOUT US
History
Established in 2000 as a 501C3 non-profit, the program was established to meet the needs of elders that live in their own homes. By enhancing and protecting the health and well being of elders, families and caregivers in our community, our program provides a means for people to improve and maintain wellness, stay in their homes they love, avert or delay nursing home placement and effectively prevent health care problems before they escalate.
Mission
The mission of LAHBNP is to assist older persons, helping them to live safely and independently in their own homes and communities. Recognizing the capability of local neighborhoods and communities to integrate the social and in-home services needed by the elderly and disabled, our goal is to help seniors and disabled people avoid unnecessary emergency room visits and premature entry into the long term care setting.
Vision
In meeting its mission, the LAH/BNP draws upon a network of wonderful, dedicated volunteers under the direction of three part-time staff. LAH/BNP’s Board of Directors is made up of 9 members who bring expertise in health care, finance, administration, education and ministry to their oversight of the program. We serve people 60 years of age or older and with disabilities in Granite Falls and the surrounding rural community in SW Minnesota. GF-LAH/BNP coordinates with several area organizations and agencies in order to reach the most isolated and vulnerable.

Meet the team
Kim Kranitz-Savig, RN
Volunteer Coordinator
Born and raised in Granite Falls, I am lucky to call this home. Through the years I have spent 42.5 years working as a registered nurse at the Granite Falls Municipal Hospital and Manor in Granite Falls and in Clarkfield—I LOVE being a nurse and caring for and meeting so many WONDERFUL people. It is nice to be part of The Living At Home/Block Nurse Program! If you think you might like to join the work we do supporting our wonderful seniors, give me a call! I look forward to visiting with you and would be happy to welcome you to the team!
Lisa Putz
My name is Lisa Putz. I live in Granite with my husband and 2 children. I have lived and worked in this area for almost 20 years, moving here from Vesta where I grew up. I have spent years working with the public in all different settings. In my spare time I enjoy baking, camping and photography. I love helping people and I’m so excited
to be a part of the LAHBNP team. I have already met so many awesome people and look forward to meeting all the wonderful clients andvolunteers.
Mark Mark Roisen
Executive Director
I grew up on our family’s farm near Lac qui Parle lake, where I still live and farm part-time. I have one son who is married and currently lives in the metro area. I earned my college degree at the University of MN, moving back to the farm in, 1983. I worked 15 years as the Executive Director for the Lac Qui Parle Health Network, a hospital services cooperative before joining the GF-LAH/BNP in, 2016. It is a pleasure and a privilege to be working with staff, volunteers and the Board of Directors for the GF-LAH/BNP, providing essential services and programs for the seniors and disabled people living in Granite Falls and surrounding area.
